The 12 Simple Steps To Register

There are 12 simple steps to authorize USA Food Agents to register your facility with FDA and designate USA Food Agents as your U.S. Agent (if necessary). Note: You will be providing information to USA Food Agents, and USA Food Agents will then register your facilities with FDA.

Contract & Credit Card Information

1. Click the “Proceed” button at the bottom of this page.
2. Read the contract and “sign” by clicking in the “Yes, I agree” box at the bottom and filling out the contact information requested.
3. Determine how many facilities you will be registering through USA Food Agents and enter that number along with your credit card/billing information. You will have the opportunity to choose a username and password to keep your facility information private. If you have a Business Partners code, there is a space provided for you to enter it on the form.
4. Click the “Process My Transaction” button. At this point your information will be securely transferred and your credit card will be billed. (We accept Visa, MasterCard and AMEX.) A page detailing the success or failure of the transaction will be displayed. If successful, the page will also indicate your Customer ID and an individual number for each facility you intend to register. Please Note: These numbers are for USA Food Agents use only and are NOT your FDA registration numbers. We will supply you with your FDA facility registration numbers as soon as we receive them from FDA. Print this page for your records. You will also be emailed a receipt. At the bottom of the page will be a link to the Facilities Management Area. You can also access this area from the home page of our website.

Entering Your Registration Information

5. Once you click on the Facilities Management Area link, a page will appear asking for you to enter the username and password you set up in step 3, as well as your assigned Customer ID.
6. Once your login information is verified, you will be shown the Facilities Management Area page. Each facility will be listed separately with the sections of the form that are required to be filled out beneath it. Note that you may not be required to fill out all sections of the form. These sections are marked as “Not Required.” Each facility is displayed with its USA Food Agents Identification Number - please note this is NOT the FDA registration number. If you need to contact us regarding a facility prior to receiving your FDA registration number, you can use the USA Food Agents Identification Number to identify the facility.
7. Starting with section 1, click on “Enter Information” and begin entering data. Once completed, press the “Submit” button to return to Facilities Management Area page. After you have entered all of the needed data for a section, the system will inform you that section is “Complete.” Sections marked as “Incomplete” are required to be filled out before your registration can be submitted to FDA.
8. Proceed to fill in all of the sections asking you to “Enter Information.” At any time before you tell the system that the facility's registration information is complete (see Step 10), you can go back and edit the information in any sections previously completed by clicking on the “Review Information” link.

Reviewing Your Information & Submitting It To USA Food Agents

9. Once all required sections are marked “Complete,” a “Preview Registration Information” button will appear at the end of the facility's listing. Click the button to see a summary of all the information you have entered.
10. If all of this information is correct, mark the box at the end of the summary indicating that the information is correct to the best of your knowledge and that you are designating USA Food Agents to submit your registration to FDA, then click the “Submit to FDA” button. If you need to make any changes to the information first, mark the box that indicates you have more changes to make before proceeding and click the “Return to Facilities Management” button to make any additional changes.
11. You will receive an email confirmation that USA Food Agents has received your registration information and will register your facility with FDA.
12. In 3-5 days you will receive an e-mail message from USA Food Agents, an FDA registration number and an FDA Personal Identification Number (PIN) for each facility we have registered on your behalf.
